Enliven Services available in Waikato

In Hamilton we are contracted by Waikato District Health Board to support older people with high needs who have been referred to us by the local Needs Assessment and Service Co-ordination (NASC) agency.

Home-based, restorative support

Enliven's home-based restorative support services are tailored to meet the needs of older people (65+) who are living in their own home.

After consultation with each client individually tailored programmes of support are developed for them by one of our Service Co-ordinators. The restorative programme that results invariably involves the integration of physical activity. Services, which can be delivered up to three times a day, seven days a week, are provided by highly trained Key Support Workers.

There is no fixed list of services, but a service package may include:

  • Goal-based planning individually tailored to meet your personal needs and what is important to you
  • Rehabilitation to assist you in regaining skills, mobility, confidence
  • Activities to ensure you stay as independent as you can
  • Physical exercises based around daily living activities to keep you physically active
  • Practical support to keep your household up and running
  • Personal care based on your individual needs
  • Regular liaison with doctors, nurses and other health professionals to make sure that you are getting the very best support and the help that is right for you.

Enliven Waikato Day Programme

Are you are looking for a way to stay in your own home longer? Would good company, laughter and stimulating, fun activities help you achieve this? The Enliven Waikato Day Programme could be just what you're looking for.

It isfocused on reconnecting older people with their communities, providing opportunities for social interaction and various activities and outings, and supporting you to achieve other goals.

The Day Programmes offers:

  • A range of activities based on clients' interests and hobbies
  • Frequent outings, as well as opportunities to join other community groups for activities
  • Transportation to and from the Day Programme, if required
  • Morning and afternoon tea
  • A choice of a hot meal and dessert OR sandwiches and fruit for lunch.

We can cater for participants who are self-referred or referred by the local Needs Assessment and Service Co-ordination agency and are:

  • are mobile enough to be collected by a driver
  • havemedical conditionsable to be looked after by Day Programme staff.

(Other criteria apply, so please check with Enliven staff for more details. Some clients may be eligible for a Government subsidy)

Cost: $35 per visit (includes meals and transport). Your first trial visit is free.

Transition Programme

The transition programme provides a solution for older people who want to live as independently as possible in their own homes again after a short period in hospital and/or rest home.

As part of this programme we will actively work with you while you are in residential care, assisting you to rebuild your capability and confidence to a level required for your return home. This involves developing an individually-tailored programme of support reflecting your needs and what is most important to you.

The aim is to support you tomake the transition back home and then to provide you with the support you need subsequently.
