Post-DIscharge support

Our post-discharge service is aimed at supporting older people for the first four to six weeks after hospitalisation.

As with the other Enliven Positive Ageing Services the support delivered is individually tailored to each clients needs and aimed at ensuring older people can remain in their own homes. 

Support may include assistance with personal care, household tasks, meal preparation and shopping. 

Clients are regularly reassessed and the level of support they receive is reduced over time, as appropriate.

The ultimate aim is to ensure that four to six weeks after being discharged from hospital clients are ready and able to live independently again.

The post-discharge support service is available in Rotorua and Taupo.

Please note: Our services are funded by various district health boards, and are therefore free to clients. However, in order to receive Enliven Positive Ageing Services older people will need to be referred to us by the local Needs Assessment and Service Co-ordination agency.
