Transition Programme

Do you need to spend some time in hospital and/or a rest home, but want to return home?

Enliven's Transition Programme may be right for you.

It provides a solution for people who want to live as independently as possible in their own homes again after a short period in hospital and/or rest home.

As part of this programme we will actively work with you while you are in residential care, assisting you to to rebuild your capability and confidence to a level required for your return home. This involves developing an individually-tailored programme of support reflecting your needs and what is most important to you.

The aim is to support you to make the transition back to your home and then to provide you with the support you need subsequently.

This service is presently available from Presbyterian Support Northern in Hamilton ,Tauranga, Rotorua and Taupo.

Please note: Our services are funded by various district health boards, and are therefore free to clients. However, in order to receive Enliven Positive Ageing Services older peoplewill need tobe referred to us by thelocal Needs Assessment and Service Co-ordination agency.
